* These physicals are for school age children in grades K-12 primarily for sports or camp activities. A parent or legal guardian must accompany all students under the age of 18 and parents must bring the appropriate documentation or paperwork from their chid's school district to be completed at the time of the visit.

Why get a sports physical?

• Sports physicals, also called pre-participation physicals, are most often required by Midland Independent School District and athletic leagues to ensure your child is healthy and ready to safely play. Sports physicals raise awareness of any health issues that may interfere with your child’s ability to participate in a sport. Cornerstone Family Health providers can review past injuries, identify areas that may be prone to injury, and suggest tips and exercises to avoid future problems. We will check your athlete’s vitals, pulse, blood pressure, height, weight, vision, lungs, abdomen, ears, nose, throat, and medical history. We’ll complete the forms required by your school district. A sports physical is the first step in getting your athlete ready for the games ahead.

Does Insurance Cover the Physical?

• We conduct your child's sports physicals at the same time as their Well-Child exam and bill as they are intended to be per the guidelines of the American Medical Association and CPT. Billing the visit does not guarantee payment from your insurance company, which is ultimately decided by the plan's benefits. Most insurance companies cover your child's well-child exam at 100% one time every 365 days, so no payment is due at time of service for this exam.  It's a perfect way to kill two birds with one stone. It will be the parents responsibility to verify if your child has already had their once a year well-child exam, if you don't you may receive a statement from us stating your insurance carriers decided that the billed amount should be the patients responsibility. 

• Due to this - many other facilities may offer a flat fee sports physical and do not bill your insurance at all.  Some of these facilities may try to get you in and out quickly, which may be convenient for you on time, but they may not be very  thorough and therefore could be harmful if something is overlooked, which is not in the best interest of your child.

What if I don't have insurance or my child already had a well-child exam for the year?

• If you insurance coverage isn't available to you and your family, be sure to check out our Direct Primary Care (DPC) Membership Options.  Our DPC is a low cost alternative to expensive major medical insurance allowing our DPC exclusive access to our Board Certified Nurse Practitioner's with No Copay's EVER.  If your'e a DPC member your Yearly Well-Child exam & Sport's Physical is FREE.

• We offer discounted pricing for our Self-Pay patients School Sports Physical fo $50, which still includes an in-depth physical to ensure your child's health and well being is at top of mind during the examination.

• If your child already had their well-child exam, we would be happy to forgo your insurance and charge your our self-pay price for the exam/sports physical of $50.00.

• ​

• The Self-Pay price does not include an Office Visit to discuss additional issues, such as sickness, illness or any ancillary services or testing such as covid test, flu/strep testing etc.  If you need to discuss those types of issues the visit can be billed to your insurance company as an Office Visit and copay or deductible will be due at time of service.

• For Self-Pay patients, if an Audiogram hearing test is required, an additional $30 will be charged bringing the total cost to $85.
